How to Get Pag IBIG MDF Copy Online: A Step-by-Step Guide

Would you like to obtain a copy of your completed Pag-IBIG Member’s Data Form (MDF) for business or employment purposes? The Pag-IBIG Member’s Data Form (MDF) is a required document that you must complete and submit to become a member of the Home Development Mutual Fund (HDMF), commonly known as the Pag-IBIG Fund. The Pag-IBIG MDF contains personal information, addresses, contact details, employment history, and other relevant data of the member. It also includes a 12-digit Pag-IBIG Membership ID (MID) number and a Registration Tracking Number (RTN).

These days, registration as a Pag-IBIG Fund member can be completed online. Prospective members no longer need to fill out and submit the Pag-IBIG MDF as they can input their information directly on the Pag-IBIG online registration website. This method is more convenient and easier compared to manually filling out forms and submitting them in person at the Pag-IBIG office.

However, you may still use the Pag-IBIG MDF to apply for to get  Pag-IBIG mdf copy on line number if you are unable or prefer not to register through the Pag-IBIG website. The Pag-IBIG Fund still accepts applications using this method. You can download the Pag-IBIG MDF and print it on short bond paper or obtain one from the nearest Pag-IBIG office.

How to Fill Out Your Pag-IBIG MDF

To become a member of the Pag-IBIG Fund, you can apply by completing the Pag-IBIG Member’s Data Form (MDF) and submitting it to the nearest Pag-IBIG branch. Here are the steps to fill out the Pag-IBIG MDF:

  1. Complete the form using one (1) copy only.
  2. Write all entries in BLOCK or CAPITAL LETTERS only.
  3. Fields marked with an asterisk (*) are mandatory.
  4. If the information does not apply, write N/A.

It is important to note that the form must be completed in English.

Occupational Status

If you currently have a job or work for yourself, please select the “Employed” box. If you are not currently employed or seeking employment for the first time, please select the “Unemployed/Not Yet Employed” box. For first-time job seekers, please also select the box below.

Membership Category

  1. Mandatory: Membership which is required or compulsory.
  2. Employed (Private): Membership for individuals working for a private employer, regardless of whether they are already registered with the Social Security System (SSS) or not. This includes foreign employers with an administrative agreement with the Pag-IBIG Fund.
  3. Employed (Government): Membership for individuals working for a government office that is covered by the Government Service Insurance System (GSIS). This includes uniformed personnel of the Armed Forces of the Philippines (AFP), Philippine National Police (PNP), Bureau of Fire Protection (BFP), and Bureau of Jail Management and Penology (BJMP), as well as members of the judiciary and Constitutional Commissions.
  4. Employed Private Household: Membership for individuals working in a private household (e.g. domestic helper).
  5. Overseas Filipino Worker (OFW): Membership for individuals working for a foreign-based employer, whether based on land or at sea.
  6. Self-Employed: Membership for individuals engaged in a business, trade or profession with a monthly income of at least ₱1,000, not over 60 years old, and not under an employer-employee relationship. This includes the following sub-categories:
  7. Professional/Business Owner: Membership for individuals earning income through their own business or profession.
  8. Job Order Personnel: Check this box if you are hired to perform a specific task or short-term work of fewer than 6 months, and you are paid on a daily or hourly basis.
  9. Other Earning Group (OEGs): This refers to small-scale units that produce goods and services meant to generate income. Examples of OEGs are farmers, fishermen, public utility transport drivers, and market vendors.

Voluntary

The Pag-IBIG Fund offers voluntary membership to individuals who are not required or mandated to become members. Here are the different categories of voluntary Pag-IBIG Fund membership:

Employed (Foreign Government) – This category is for employees of foreign governments, such as those working in embassies and consulates, or international organizations that do not have an administrative agreement with the Pag-IBIG Fund.

  1. Barangay Official/Employee – If you hold a position of authority in your barangay, you can check this box to indicate your membership category.
  2. Non-Working Spouse – If you are a full-time homemaker and manage household and family affairs, you can opt for this membership category.
  3. Member of Religious Group – If you are a member, head, or leader of an organization that exercises religious beliefs, you can choose this category.
  4. Pensioner/Investor/Lessor – This membership category includes individuals who receive an old-age or permanent total disability pension, those who own securities or shares (including REITs or their derivatives), and those who own, administer, or represent a residential unit for lease.

  • Cooperative/Trade Union Member – Select this option if you are a member of a cooperative or a trade union.
  • Employer/Business Address: Write the complete address of your employer or business, as indicated in your registration certificate or employment cont
  • Office Assignment: Indicate whether you were assigned to the head office or a particular branch. If you were assigned to a branch, specify the location.
  • From: Indicate the month and year (MM/YYYY) you started working for the employer or business. For example, if you began working in July 2020, write 07 2020.
  • Last Name: The surname or family name of your heir
  • First Name: Your heir’s given name
  • Name Extension: If applicable, indicate any suffix such as Jr., III, IV
  • Middle Name: This is usually the mother’s maiden surname, or in the case of married women, the father’s surname
  • No Middle Name: Check this box if your heir doesn’t have a middle name, such as an illegitimate child

Signature of Informant – Please affix your signature on the space provided. Date – Please write the date when you accomplished the MDF (e.g. May 10, 2022).

After accomplishing the Pag-IBIG MDF, kindly submit it to the nearest Pag-IBIG branch during office hours, which are from 8:00 AM to 5:00 PM, Mondays to Fridays (except holidays).

How to Get Pag-IBIG MDF for Existing Members

Currently, obtaining a copy of your Pag-IBIG MDF online is not yet available. You will need to personally visit the nearest Pag-IBIG Fund office to make a request for a copy of the form.

To request for a hard copy of your Pag-IBIG MDF that indicates your Pag-IBIG MID number, please bring the following requirements:

  • Original and photocopy of a valid ID that is acceptable by the Pag-IBIG Fund
  • Duly accomplished Pag-IBIG Fund Request Slip (HQP-PFF-369)

You can download the Request Slip by clicking the provided link. Once downloaded, open the PDF file and print it on long bond paper (8″ x 13″). If long bond paper is not available, you may use short or A4 paper as long as you set the size to fit.

Note that you will need to visit a Pag-IBIG branch personally to make the request.

To make a request for a copy of your Pag-IBIG MDF, please fill out the Request Slip provided and follow these steps:

  1. Requestor’s Name – Write the complete name of yourself (or the Pag-IBIG member).
  2. Date of Release – This field can be left blank.
  3. Pag-IBIG MID No. – Indicate your 12-digit Pag-IBIG Membership ID number. If you forgot or lost your Pag-IBIG MID number, please refer to this article for guidance.
  4. Employer ID No. – Specify the Pag-IBIG Employer ID number of your current employer as stated in the Employer’s Data Form (EDF).
  5. Purpose – Write the purpose of your request (e.g. “For employment”).
  6. Particulars – In the Particulars column, check the box for “Member’s Data Form (MDF, system-generated).”
  7. No. of Copies – In this column, indicate how many copies of the Pag-IBIG MDF you require.
  8. Date – Write the date the request was made.

After filling out the Request Slip, go to the nearest Pag-IBIG branch and submit it along with a photocopy of a valid ID. You must also bring the original ID for authentication purposes.

Note that Pag-IBIG branches are open from 8:00 AM to 5:00 PM, Mondays to Fridays (except for holidays).

Overseas Filipino Immigrant – If you are a Filipino citizen or permanent resident living outside of the Philippines, please check this option.

Personal Details

To complete the “Member” field, please follow the steps below:

  • First Name: Enter your given name.
  • Name Extension: If your name includes a suffix (e.g. Jr., III, IV), please indicate it here.
  • Middle Name: Enter your mother’s maiden surname. For married women, enter your father’s surname.
  • No Middle Name: Check this box if you do not have a middle name (e.g. illegitimate child).

For the fields “Father,” “Mother,” and “Spouse,” please follow the instructions above for “Member” in writing the last name, first name, name extension, and middle name.

  • Write your date of birth in the MM/DD/YYYY format.
  • Indicate the city/municipality/province where you were born. If you were born outside of the Philippines, include the country.
  • Check the appropriate box for your sex, whether male or female.
  • State your height in centimeters (cm), using a height converter if necessary.
  • Indicate your weight in kilograms (kg), using a weight converter if necessary.
  • If you have a Common Reference Number (CRN), write it down.
  • Check the box that corresponds to your marital status.
  • Specify your nationality; if you are Filipino, write “Filipino.”
  • If you have any prominent distinguishing facial features, such as a mole under your left eye, note it down.
  • Check the appropriate box for the frequency of membership savings if contributions are not through payroll deduction.

  • Check the box that corresponds to your marital status.
  • Specify your nationality; if you are Filipino, write “Filipino.”
  • If you have any prominent distinguishing facial features, such as a mole under your left eye, note it down.

Please provide your current occupation, profession or line of work, such as “teacher”. Indicate your employment status by selecting the appropriate option. If you are formally employed, an overseas Filipino worker (OFW), a self-employed professional, or a business owner, write the complete name of your employer or business based on your registration certificate or employment contract. Provide the complete address of your employer or business, as well as the type of work you do (for OFWs only). If you are an OFW, please check the appropriate box for your type of work, whether land-based or sea-based, and specify the country of assignment. Indicate your monthly income, including basic salary and allowances. If you are assigned to a branch, write its location on the space provided and check the appropriate box on whether you are assigned to the head office or a particular branch. Finally, write the month and year when you started working for your current employer.

To become a member of Pag-IBIG Fund, you need to accomplish the Member’s Data Form (MDF). Here are the steps on how to fill it out:

  1. Personal Information Write your complete name, gender, civil status, and birthdate. If applicable, also indicate your name as it appears on your birth certificate, as well as your previous name/s if you changed your name/s due to marriage or any other reasons.
  2. Contact Information Write your current and permanent addresses, email address, and mobile/telephone number/s. If your current and permanent addresses are the same, just write “Same” in the space provided for the permanent address.
  3. Identification Details Write your valid ID number/s and the date/s of expiration. If you have more than one valid ID, write the details of the primary ID and the secondary ID.
  4. Employment Information Indicate all your previous employers starting from the date of Pag-IBIG Fund membership. Write the complete name and address of your employer or business, your office assignment, the date/s when you started and ended your work, and your legal heirs in accordance with the Laws of Succession.
  5. Certification Read the certification carefully, affix your signature, and indicate the date when the Pag-IBIG MDF was accomplished.

After accomplishing the Pag-IBIG MDF, submit it to the nearest Pag-IBIG branch. If you need a copy of your Pag-IBIG MDF indicating your Pag-IBIG MID number, just visit any Pag-IBIG branch and bring an original and photocopy of a valid ID acceptable by the Pag-IBIG Fund and a duly accomplished Pag-IBIG Fund Request Slip (HQP-PFF-369).
